Audio CD Service On-line Request

This form is to request to receive one or more church services on Audio CD. There are three kind of 'subscriptions' available through this web form: |
- Month - If you need one month of services for any reason, such as, being sick, vacation, whatever, you may choose the 'Month' option. This will let you select the Month you desire to receicve.
- Week - If you would like to receive one week of services, usually two on Sunday, and one on Wednesday, you may choose the 'Week' option. This will let you select a date during the Week you desire to receive.
- Day - If you would like to receive one specific Sunday or Wednesday service, you may choose the 'Day' option. This will let you select the specific date of the Day you desire to receive.
